Primary pneumococcal peritonitis with a fulminant course

چکیده

Úvod: PrimĂĄrna peritonitĂ­da predstavuje zĂĄpal peritoneĂĄlnej dutiny bez jasnĂŠho intraabdominĂĄlneho zdroja infekcie. Je súčasĹĽou diferenciĂĄlnej diagnostiky nĂĄhlych prĂ­hod bruĹĄnĂ˝ch a mĂ´Ĺže byĹĽ potencionĂĄlne Ĺživot ohrozujĂşca. Pneumokoky boli častĂ˝m patogĂŠnom spĂ´sobujĂşcim primĂĄrnu peritonitĂ­du hlavne v preantibiotickej ĂŠre, v súčasnosti sĂş vyvolĂĄvajĂşcim agens skĂ´r zriedka. U dospelĂ˝ch sa pneumokokovĂĄ vyskytuje u pacientov s predisponujĂşcimi faktormi, akĂ˝m je naprĂ­klad cirhĂłza pečene ascitom. V zdravej populĂĄcii tĂĄto diagnĂłza vzĂĄcna a diagnostika preto nĂĄročnĂĄ, nevyhnutnĂŠ prvom rade vylúčiĹĽ sekundĂĄrnu peritonitĂ­du. Kazuistika: naĹĄom člĂĄnku prezentujeme prĂ­pad 36-ročnej Ĺženy, osem týŞdňov po pĂ´rode per vias naturales, ktorĂĄ bola prijatĂĄ na naĹĄe oddelenie pre nĂĄhle vzniknutĂŠ difĂşzne bolesti brucha obrazom septickĂŠho stavu. KontrastnĂĄ počítačovĂĄ tomografia (CT) abdomenu nenaĹĄla patolĂłgiu vysvetÄžujĂşcu stav pacientky. Zaviedla ĹĄirokospektrĂĄlna antibiotickĂĄ liečba a indikovanĂĄ laparotĂłmia, pri ktorej takisto nenaĹĄiel zdroj infekcie, intraabdominĂĄlne bol prĂ­tomnĂ˝ hnisavĂ˝ nezapĂĄchajĂşci vĂ˝potok, peroperačne vykonanĂĄ toaleta bruĹĄnej lavĂĄĹžou antiseptikom. KultivĂĄcia priniesla monobakteriĂĄlny nĂĄlez Streptococcus pneumoniae. Pri pokračujĂşcej antibiotickej liečbe a lavĂĄĹži pacientky chirurgickej revĂ­zii postupne znormalizoval. ZĂĄver: dospelej ĹĽaĹžko diagnostikovateÄžnĂĄ, imituje peritonitĂ­du, od musĂ­ odlĂ­ĹĄenĂĄ. Vo väčšine prĂ­padov klinickĂŠ odlĂ­ĹĄenie operačnej intervencie nemoĹžnĂŠ. ChirurgickĂĄ mĂĄ diagnostickom i terapeutickom procese primĂĄrnej pneumokokovej peritonitĂ­dy dĂ´leĹžitĂş Ăşlohu, rovnako ako sprĂĄvna terapia. rĂĄmci by diagnĂłzu nemalo zabĂşdaĹĽ.
